Confirm you’re eligible before filing

This process applies to most employer plans, Pennie plans, and plans bought directly from an insurer.

Usually, you must first complete the plan’s internal appeal and have a Final Adverse Benefit Determination.

You generally must file within 4 months of that final denial letter.

If your life or health is at serious risk, you can request an expedited review right away with physician certification.

If the denial is upheld, coverage can remain denied under this process; next practical step is to review the written rationale, explore any new/corrected evidence, and discuss alternative care and billing options.
